Healthcare Outcome Payment Effort (HOPE)

Background:
HOPE is a voluntary, upside-only outcome payment model developed by HSCRC that rewards hospitals and non-hospital partners for measurable reductions in inpatient and ER spending while improving population health. HOPE creates a clear, predictable, and accountable payment structure that enables hospitals, payers, and community partners to invest in interventions and share in savings. HOPE seeks to sustain and expand population health investments that advance AHEAD’s goals and drive meaningful system transformation.
Two Pathways for Participation:
- Regional and Statewide Initiatives (RSI)
- A pathway for a regional or statewide entities such as community-based organizations, health plans, technology companies, and other partners to collaborate with hospitals on initiatives serving regional or statewide populations.
- Care Transformation Framework (CTF)
- A hospital-led pathway for hospitals or groups of hospitals to implement hospital-defined interventions among target populations and reduce inpatient and emergency department spending.

HSCRC staff have been communicating HOPE activities and updates to stakeholders through the
Total Cost of Care Workgroup and the HOPE Workgroup. The TCOC workgroup meets on the 4th Wednesday of the month and the HOPE Workgroup meets in between the TCOC meetings. Please find the HOPE Workgroup schedule and materials below.
