Trustee Disclosure Information

As required in Section 400 - Reporting Requirements of the Health Services Cost Review Commission (HSCRC) Budget and Accounting Manual, a list of all active hospital Trustees with business addresses are to be submitted 120 days after the end of the hospital's fiscal year. An individual disclosure form of each Trustee doing business​ of ​$10,000 or more with the hospital is required to be submitted in that time period.  

Required Forms​

List of Trustees Survey - One List of Trustees is required per hospital.

Individual Trustee Disclosure Survey - This form is to be completed by only individual hospital Trustees who have had business transactions with the hospital in the amount of $10,000 or more.
