Clinical Adverse Events Measure (CAEM) Subgroup

HSCRC is convening the Clinical Adverse Events Measures (CAEM) Subgroup to the Performance Measurement Workgroup from April to August, 2026.  The overarching objective of the effort is for the Subgroup to vet and recommend updates to the hospital complications program measures and mortality measures used in our pay for performance programs for all payer patients other than Medicare FFS patients.  The need to do this is the onset of  the state AHEAD Model which began this year; the Model will transition all Maryland hospitals in CY 2028 to CMS global budgets for Medicare FFS revenue and use the federal quality programs starting with FY2029 or FY2030.  The state will continue to administer hospital global budgets and quality programs for the other payers under the Maryland HGB program.  Our redesign efforts are to align the measures as much as possible with CMS while also still focusing on and incentivizing state specific or population specific priorities (e.g., Severe Maternal Morbidity, etc.).


CAEM Goals
The CAEM Subgroup will assess opportunities for further alignment with CMS quality programs under Medicare FFS HGB, to evaluate candidate complication and mortality measures, and to advise the staff and the PMWG on a select group of measures that should be considered for use in the MHAC and QBR programs for Maryland HGB. In addition, the CAEM Subgroup will consider and finalize criteria for measure selection criteria; this will include consideration of Quality program established Guiding Principles, and the following criteria staff initially proposes:
  1. ​Alignment with CMS quality programs 
  2. Maintenance of all-payer accountability and incentives for quality 
  3. Reduction of retrospective measure evaluations to the extent possible
  4. Attention to areas of poor performance and/or priority area for State, hospitals, payers, or other stakeholders ​​
The Subgroup will work early on in the process to refine and finalize the measure selection criteria.
